Short-term Restorative Care

The District Nurses provide short-term restorative care (STRC) in the North and South of Tasmania.

STRC is a goal oriented, multidisciplinary package of care designed to meet the client’s needs to help them stay at home and help defer entry into higher levels of care.  

Access into STRC is due to functional decline (not linked to a recent hospital admission) resulting in a client needing assistance for a short period of time.  It provides a high intensity period of care, up to a maximum of 56 days.

Individual clients can access STRC twice in any 12 month period.

STRC is delivered by a care team of at least three health professionals, one of which will be a doctor (usually the clients GP).

The District Nurses will coordinate the clients STRC, and the package will not affect any services the client currently receives.  There is no cost to the client for STRC.

 

The types of services a client may receive include, but is not limited to:

aids and equipment

physiotherapy

occupational therapy

footcare

clinical nursing

personal care and assistance

meal preparation, nutrition and diet advice

personal medical alarm
