Restorative Care Pathway

The District Nurses provide short-term restorative care (STRC) in the North and South of Tasmania.

RCP is a goal oriented, multidisciplinary package of care designed to meet the client’s needs to help them stay at home and help defer entry into higher levels of care.  

Access into RCP is due to functional decline (not linked to a recent hospital admission) resulting in a client needing assistance for a short period of time.  It provides a high intensity period of care, up to 12 weeks, with a possible 4 week extension.

Individual clients can access RCP twice in any 12 month period.

RCP is delivered by a care team of at least three health professionals, one of which will be a doctor (usually the clients GP).

The District Nurses will coordinate the clients RCP, and the package will not affect any services the client currently receives.  There is no cost to the client for RCP.

 

The types of services a client may receive include, but is not limited to:

aids and equipment

physiotherapy

occupational therapy

footcare

clinical nursing

personal care and assistance

meal preparation, nutrition and diet advice

personal medical alarm
