Upcoming changes to Home Care Packages:

The Australian Government is introducing a new approach to aged care funding with changes expected to take effect from 1 November 2025. These changes will see the existing Home Care Packages and Short-Term Restorative Care programs replaced by a new program called Support at Home.

If you are signed up to a Home Care Package before 1 November 2025, you will automatically transition over to the Support at Home program when the changes come into effect.

HOME CARE PACKAGES

There are four levels of home care packages:

When you deal with The District Nurses you always speak to someone who lives in Tasmania who can meet you face to face, anytime, anywhere.

To arrange an assessment for a Home Care Package, please contact My Aged Care. Our Home Care Packages are co-ordinated by Registered Community Nurse Navigators who are specialists in linking services to clients.  They can assist you to contact My Aged Care.

The District Nurses employs its own Nurses and Community Support Workers in Northern and Southern Tasmania, guaranteeing clients the highest quality of care.

Unlike other providers, The District Nurses will not charge you an exit fee should you need to change to another Home Care Package Provider.

Aged Care service offerings

Keeping you healthy.

Community Nursing

Community Nursing services are provided in your home and in our Community Clinic by a qualified nurse and may be on a regular or occasional basis. Services provided include:

  • wound care
  • medication management
  • blood pressure monitoring
  • continence and catheter management
  • diabetes management
  • stomal therapy
  • End of life/palliative care
  • other specialised nursing related activities

When we have received your referral from My Aged Care for people over 65 years or Tasmanian Community Care Referral Service for under 65 years, one of our staff will make a time to talk to you.

Our nursing assessor will develop a care plan with you that details the nursing assistance that you need.

All of our nursing services can also be purchased privately through The District Nurses Private Services for a fee and can be provided 7 days per week, 365 days of the year.

Social Support
Enabling you to do the things you want to do

Social Support

We can provide you with the services and supports you want to assist you to get out of the house and enjoy time with others.

This could include:

  • Visiting family and friends
  • Shopping
  • Bill paying
  • Attendance at appointments

When we have received your referral from My Aged Care for people over 65 or from the Tasmanian Community Care Referral Service for people under 65, one of our staff will make a time to talk to you.

Our assessor will work with you to develop a care plan which details the social support activities that will meet your needs and goals. Changes to these support activities can be negotiated at any time.

All of our services can be purchased privately through The District Nurses Private Services and can be provided 7 days per week, 365 days of the year.

Care that is right and safe for you

Personal Care

We can provide safe and effective personal care that is delivered in line with your needs, goals and preferences and with a focus on your health and well-being.

Activities can include support with:
  • Eating
  • Bathing
  • Toiletry
  • Dressing
  • Grooming
  • Getting in and out of bed
  • Moving about the house

When we have received your referral from My Aged Care for people over 65 or from the Tasmanian Community Care Referral Service for people under 65, one of our staff will make a time to talk to you.

Our assessor will work with you to develop a care plan which details the social support activities that will meet your needs and goals. Changes to these support activities can be negotiated at any time.

Helping you keep things running.

Home Maintenance

We are happy to consider any minor household maintenance which will assist clients maintain safety and independence at home.

These can include:
  • repairs to window/door locks
  • repairs to gates
  • changing light globes
  • replacing smoke detector batteries
  • turning the mattress
  • changing tap washers
  • repairs to clotheslines

We can also provide lawn mowing and garden pruning. When we have received your referral from My Aged Care, one of our staff will make a time to talk to you. Our assessor will develop a care plan with you that will detail the assistance that you would like. All of our services can be purchased privately through The District Nurses Private Services for a fee and can be provided 7 days per week, 365 days of the year.

Enabling you to manage everyday activities in a safe, secure and healthy home environment

Domestic Assistance

We can provide you with the services and supports within your home that will help you to do all the things that you want and are able to do within your home and provide assistance with the more difficult tasks.

Activities can include:

  • Dishwashing
  • House cleaning
  • Clothes washing and ironing
  • Unaccompanied shopping
  • Unaccompanied bill paying
  • Help with meal preparation
