Changes to Home Care Packages:
The Australian Government has introduced a new approach to aged care funding with changes expected to take effect from 1 November 2025. These changes will see the existing Home Care Packages and Short-Term Restorative Care programs replaced by a new program called Support at Home.
If you were signed up to a Home Care Package before 1 November 2025, you will automatically transition over to the Support at Home program when the changes come into effect.

To arrange an assessment for a Support At Home program package, please contact My Aged Care. Our Support At Home program packages are co-ordinated by Registered Community Nurse Navigators who are specialists in linking services to clients. They can assist you to contact My Aged Care.
The District Nurses employs its own Nurses and Community Support Workers in Northern and Southern Tasmania, guaranteeing clients the highest quality of care.
Unlike other providers, The District Nurses will not charge you an exit fee should you need to change to another Support at Home Provider.
