End of Life Care

End of Life Care Supplementary Services

The EoLCSS is for those clients who may need some additional supports so that they can be cared for at home in the last months of life.

Based on a palliative approach to care which is client focused and inclusive of family and carers, services that can be offered include, but are not limited to the following:

Personal care

Overnight sits

Respite

Nursing interventions where required

This program is funded by the Department of Health (Tasmania)

Referrals from Health Care Professionals can be made by using the Referral for EoLCSS.
