Commonwealth Home Support Program

What is CHSP?

The Commonwealth Home Support Program provides a range of entry level services for people who need assistance to keep living independently at home and in their community. It is designed to provide small amounts of a single service or a few services to a large number of older people who need only a small amount of assistance to enable them to maintain their independence.  It is about building on your strengths and abilities to help you continue living safely in your home and participating in your community.  We will work together with you to develop a care plan that addresses what is important to you, your likes, needs and preferences.  You are at the centre of your care.

The District Nurses support clients through the Commonwealth Home Support Program (CHSP), providing:

Nursing

Personal care eg assistance with showering and dressing

Domestic assistance eg house cleaning, washing clothes

Social Support

Help with shopping

Gardening and minor maintenance

Allied health (exercise and wellness programs)

The fees charged by The District Nurses for CHSP services are designed to ensure that they are affordable for all members of the community.

Pension & Health Care Card Holders 

Monday – Friday$10.00 per visitCHSP Personal Care, Social Support, Respite, Transport, Home Maintenance
Monday – Friday $15.00 per visit Domestic Assistance
Monday – Sunday$15.00 per visitNursing
 Additional visits per week are capped at $35.00.

Non Pension & Non Health Care Card Holders 

Monday – Friday$20.00 per visitCHSP Personal Care, Nursing, Social Support, Respite, Transport, Home Maintenance, Domestic Assistance
Saturday/Sunday$20.00 per visit Nursing only
Public Holidays $20.00 per visitNursing only
 Additional visits per week: $10.00, capped at $30.00.

(Capped prices are the maximum amount you will pay per week including additional visits)
