We offer two types of aged care services, depending on your funding:
Commonwealth Home Support Program (CHSP) — for those needing occasional help with daily tasks
Support At Home Program (SAH) — for those needing more regular or complex support

The Commonwealth Home Support Program provides a range of entry level services for people who need assistance to keep living independently at home and in their community.
Support At Home Program (SAH)
We are an approved provider of Aged Care under the Commonwealth Aged Care Act providing a full range of services across Tasmania.

What aged care services does The District Nurses provide?
The District Nurses provides a range of in-home aged care services across Tasmania, including nursing, personal care, domestic assistance, social support, medication assistance, home maintenance and restorative care programs.
Who can access aged care services?
Older Australians who require support to continue living safely and independently at home may be eligible for aged care services. Eligibility is generally determined through a My Aged Care assessment.
What is the Support At Home Program?
The Support At Home Program is an Australian Government aged care program designed to help older people remain independent at home through coordinated care and support services.
What is Short-Term Restorative Care (STRC)?
Short-Term Restorative Care is a goal-oriented program that helps older Australians improve independence and wellbeing after illness, injury or functional decline. Services may include physiotherapy, occupational therapy, nursing and personal care.
How do I apply for aged care services?
To access government-funded aged care services, you can contact My Aged Care to arrange an assessment. The District Nurses team can also help guide you through the process.
Can I receive nursing care at home?
Yes. The District Nurses provides community nursing services that may include wound care, medication assistance, chronic condition support, continence care and clinical monitoring in the home.
Are services available privately?
Yes. Services can also be purchased privately through The District Nurses Private Services, including nursing, personal care and in-home support.
