Once the referral has been received by Care Point a consultant shall phone the client to conduct an assessment over the phone. 

The referral is faxed to The District Nurses; members of the staff shall contact the referred client to arrange a time for a full assessment of service needs (new clients) or services reactivated for those who have been admitted to hospital.
